Job Description PHP is seeking a travel nurse RN Home Health Care Manager for a travel nursing job in Clearlake, California. Job Description & Requirements - Specialty: Care Manager - Discipline: RN - Start Date: 07/22/2025 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel 438112 THIS IS FOR HOME HEALTH NOT HOSPITAL. Home Health experience is A MUST/REQUIRED. MUST have HCHB software experience is required. Job Summary: Provides excellent patient care by assisting in collaboration, development, implementation, revision and reporting of the case management program. Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient's healthcare benefactor to optimize outcomes. Serves as a consultant to the healthcare team on specific patient items. Job Requirements: Education and Work Experience: Bachelor's Degree in Nursing (BSN): Preferred Experience in a care management role: Preferred Licenses/Certifications: Registered Nurse (RN) licensure in the state of practice: Required Case management certification: Preferred Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required About PHP At PHP, you are always the first focus!
