Job Description:
MindPath Care Centers, a pioneering network under Community
Psychiatry, seeks a dedicated and compassionate Psychiatric Mental
Health Nurse Practitioner (PMHNP) to join our team. At MindPath, we
provide focused outpatient mental health treatment targeting a
variety of patient needs. Leveraging therapy and medication
management tailored to individual needs, we aim to foster a
supportive and healing environment. As a PMHNP, you will play a
crucial role in the diagnostic evaluation, treatment planning, and
management of psychiatric disorders for adults. Join us in our

patient-centric environment. Conduct comprehensive patient
assessments to gather medical history and psychiatric conditions.
Develop individualized care plans integrating medication management
and psychotherapeutic treatments. Monitor patient progress and
adjust treatment plans as necessary. Collaborate with a
multidisciplinary team to ensure continuity of care. Provide
psychoeducation to patients and their families to enhance
understanding and engagement in the treatment process. Maintain
accurate patient records and adhere to privacy regulations.
Expertise in psychiatric evaluation and medication management.
Strong interpersonal and communication skills. Ability to work
collaboratively in a multidisciplinary health setting. High level
of empathy and professionalism. Medical Insurance Dental Insurance
Vision Insurance Life Insurance 401k Active Nurse Practitioner
license in the state of practice. Board certification as a
Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC).
